A Case of Immune Thrombocytopenia Associated with Diffuse Histiocytic Lymphoma




Abstract
The reports for the immune thrombocytopenia complicated with lymphoproliferative
disorders and carcinomatous diseases were occasionally issued.
We reported a case of 32-yr-old female patient with Diffuse Histiocytic Lymphoma
(DHL), complicated with immune thrombocytopenia which was confirmed by immuno-
fluorescent technique.
She was admitted to the hospital because of dyspnea.
A chest roentgenogram showed a lobulated mass shadow in superior anterior media-
stinum. On abdominal examination, a round mass with surface nodularity was palpated
in right, hypochondrial and epigastric area. Biopsy of supraclavicular lymph node showed
diffuse histiocytic type of malignant lymphoma (Rappaport Classification). Initial
laboratory studies showed a platelet count of 73, 000/mm3. Examination of
the bone marrow showed increased megakaryocyte and antiplatelet antibody was positive
result with immunofluorescent technique.
She was treated with the chemotherapy of BACOP schedule (Cytoxan, Adriamycin,
Vincristin, Bleomycin, Prednisolone) under the diagnosis of DHL. ¥², with transient
remission, but the platelet counts showed lower than 30, 000/mm3
throughout the hospital course.
